Insurance Code section 11520.5
(a)
Upon the holder’s ceasing to exist as a separate entity.(b)
Upon the winding up or dissolution, or expiration or forfeiture of the corporate existence of a corporate holder thereof.(c)
Upon winding up or dissolution of a holder not a corporation.(d)
In any event upon surrender by the holder of its certificate of authority and cancellation of the same by the commissioner.(1)
Two hundred eighty-three dollars ($283) for each amended certificate of authority caused by a change of the name of the holder.(2)
Two hundred twelve dollars ($212) for the services and expenses of the commissioner in connection with the filing of amended articles by a holder.(3)
Eight hundred forty-nine dollars ($849) for all services and expenses of the commissioner in connection with the withdrawal of a holder of a certificate of authority under this chapter.(e)
Upon the receipt of a notice of filing of a petition by or against a certificate holder under the United States Bankruptcy Code for bankruptcy or reorganization, the commissioner shall cease imposing, billing, or collecting the annual fees due under this chapter and this section to the certificate holder.(f)
Upon notice of the suspension of the corporate status of the certificate holder for a period of 12 months by the Secretary of State, the commissioner shall terminate the certificate of authority and shall deem the certificate to be terminated.
