Health and Safety Code section 1358.22

(a)

On or before March 1 of each year, an issuer shall report the following information for every individual resident of this state for which the issuer has in force more than one Medicare supplement contract:

(1)

Contract number.

(2)

Date of issuance.

(b)

The items set forth above shall be grouped by enrollee.
