Food and Agricultural Code section 35012

An application for a milk products plant license shall be made on a form which is prescribed by the director. It shall state all of the following:

(a)

The name and address of the applicant.

(b)

The address of the milk products plant.

(c)

Such other information as to the nature of the applicant’s business as the director may require.
