CA Health & Safety Code Section 1743.27

(a)

Any license revoked pursuant to this chapter may be reinstated pursuant to Section 11522 of the Government Code.

(b)

Any licensee may, with the approval of the department, surrender his or her license for suspension or cancellation by the department. Any license suspended or canceled pursuant to this section may be reinstated by the department on receipt of an application showing compliance with the requirements of Section 1743.7.
