Health and Safety Code section 11839.19

(a)

The department shall not license the establishment of a narcotic treatment program without a written application by the treatment facility that meets evaluative criteria required by the department.

(b)

The department shall not require disclosure of the identity of patients or former patients or of any records containing identifying information except as provided in Section 11845.5.
